Responsibilities
- Deliver personalized care plans including medication management and mobility assistance
- Monitor vital signs and document client progress in our telehealth portal
- Assist with daily living activities (bathing, dressing, meal preparation)
- Coordinate with healthcare teams via secure remote communication tools
- Provide companionship and emotional support to elderly and disabled clients
- Ensure safe home environments and implement fall prevention measures
- Transport clients to appointments using company-provided vehicle
Qualifications
- Minimum 1 year professional home health aide experience
- Valid California HHA certification and CPR/First Aid certification
- Reliable transportation with clean driving record
- Proficiency with telehealth platforms and digital documentation
- Ability to work independently while maintaining client safety protocols
- Clear criminal background check and TB test
- Strong communication skills in English (Spanish bilingual preferred)
