Responsibilities
- Conduct comprehensive telehealth assessments via phone/video to determine patient acuity and care priorities
- Provide evidence-based medical advice and health education to diverse patient populations
- Coordinate care transitions between virtual services and Houston-area healthcare facilities
- Maintain accurate electronic health records while adhering to HIPAA compliance standards
- Collaborate with multidisciplinary teams to ensure continuity of care
- Participate in continuous quality improvement initiatives for triage protocols
- Manage high-volume patient inquiries during peak hours with exceptional efficiency
Qualifications
- Active RN license in Texas (TX RN) with minimum 3 years clinical experience
- BLS certification and ACLS preferred
- Proven expertise in telephone triage protocols and emergency assessment
- Experience with telehealth platforms (e.g., Teladoc, Amwell) or similar EHR systems
- Exceptional communication skills with ability to de-escalate sensitive situations
- Ability to work independently while maintaining collaborative team dynamics
- Strong clinical judgment and decision-making capabilities in virtual environments
- Spanish bilingual proficiency highly desirable
