Responsibilities
- Conduct thorough phone/video assessments to determine patient acuity and appropriate care level
- Provide expert clinical guidance to patients experiencing urgent/non-urgent health concerns
- Accurately document patient encounters in electronic health records (EHR)
- Collaborate with providers to coordinate care transitions and referrals
- Adhere to clinical protocols while maintaining empathetic patient communication
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Colorado (CO)
- Minimum 3 years acute care or emergency nursing experience
- BLS certification; ACLS preferred
- Proven triage experience in telehealth or call center setting
- Exceptional assessment and critical thinking skills
- Proficiency with EHR systems and telehealth technology
- Ability to work independently in a remote environment
