Responsibilities
- Conduct comprehensive telehealth assessments for patients presenting with acute and chronic conditions
- Provide clinical recommendations and coordinate appropriate care pathways
- Collaborate with multidisciplinary teams to ensure seamless patient transitions
- Document encounters accurately in EHR systems while maintaining HIPAA compliance
- Manage high-volume call triage with exceptional judgment and empathy
- Participate in quality improvement initiatives and training programs
Qualifications
- Active RN license in Louisiana with minimum 2 years clinical experience
- BLS certification; ACLS preferred
- Strong assessment skills in emergency and urgent care settings
- Proficiency with telehealth platforms and EHR systems
- Exceptional communication and crisis management abilities
- Experience with remote work protocols and self-management
