Responsibilities
- Conduct virtual patient assessments and monitor health status via secure telehealth platforms.
- Document patient interactions, symptoms, and care plans accurately in Electronic Health Records (EHR).
- Communicate effectively with physicians, specialists, and patients to coordinate care plans.
- Provide education and resources to patients regarding chronic disease management and wellness.
- Escalate urgent medical issues to the on-call nursing staff or physicians immediately.
- Maintain strict confidentiality and HIPAA compliance in all remote interactions.
- Participate in ongoing training and professional development to stay updated on best practices.
Qualifications
- Current RN, LPN, or CNA license in the State of New Mexico.
- Experience with telehealth, remote patient monitoring, or home health is highly preferred.
- Strong computer literacy and proficiency with Microsoft Office Suite and EMR systems.
- Excellent communication skills, both verbal and written.
- Ability to work independently in a remote environment with high self-motivation.
- CPR and BLS certification (or willingness to obtain).
- Reliable high-speed internet connection and a dedicated workspace.
