Responsibilities
- Conduct virtual patient assessments and develop personalized care plans
- Provide telehealth consultations for chronic disease management
- Coordinate care transitions between in-person and virtual settings
- Document clinical encounters in HIPAA-compliant EHR systems
- Collaborate with multidisciplinary teams for holistic patient care

- Maintain strict adherence to telehealth protocols and state regulations
Qualifications
- Current, unrestricted RN license in Florida
- Minimum 2 years clinical nursing experience
- Telehealth or virtual care experience preferred
- Strong computer literacy and tech-savvy
- Exceptional communication and assessment skills
- Ability to work independently in a remote setting
- Valid BLS certification
- Experience with EHR systems (Epic preferred)
