Responsibilities
- Deliver virtual patient assessments and care coordination via telehealth platform
- Monitor patient vitals and medication adherence remotely using digital tools
- Provide empathetic health education and emotional support to patients/families
- Collaborate with RNs and providers to create personalized care plans
- Document patient interactions accurately in EHR systems
- Respond to patient inquiries within SLA timeframes
- Participate in virtual team huddles and training sessions
Qualifications
- Active Ohio CNA certification required
- 1+ years clinical experience in home health or hospital setting
- Proficiency with telehealth technology (training provided)
- Strong written communication skills for virtual interactions
- Ability to multitask in fast-paced remote environment
- Valid Ohio driver's license for occasional local travel
- High-speed internet and quiet home workspace
