Responsibilities
- Deliver virtual patient consultations via telehealth platform
- Develop and implement personalized care plans for chronic disease management
- Coordinate with specialists and care teams for integrated patient outcomes
- Document patient encounters accurately in EHR systems
- Provide health education and medication management support
- Participate in quality improvement initiatives and protocol development
- Respond to urgent patient inquiries within service level agreements
Qualifications
- Active RN license in New York State
- BSN degree (MSN preferred)
- 3+ years clinical nursing experience
- Telehealth or virtual care experience required
- Proficiency with EHR systems (Epic/Cerner preferred)
- Strong critical thinking and decision-making skills
- Excellent communication and digital literacy abilities
- Ability to work independently with minimal supervision
