Responsibilities
- Conduct virtual patient assessments and health consultations
- Develop and implement personalized care plans
- Coordinate care with multidisciplinary teams
- Provide telehealth education to patients and families
- Document patient encounters in EMR systems
- Monitor chronic conditions remotely
- Participate in quality improvement initiatives
Qualifications
- Active Oklahoma RN license (compact license accepted)
- BSN or higher degree preferred
- 2+ years clinical nursing experience
- Telehealth or case management experience
- Proficient in EHR systems (EPIC preferred)
- Strong critical thinking and communication skills
- Ability to work independently with minimal supervision
- BLS certification required
