Responsibilities
- Provide virtual patient assessments and care coordination via telehealth platforms
- Collaborate with multidisciplinary teams to develop and implement patient care plans
- Monitor and interpret patient data from remote monitoring devices
- Document clinical information accurately in electronic health records
- Educate patients and families on health management and medication protocols

Qualifications
- Active RN license in Massachusetts
- BSN degree with 2+ years clinical nursing experience
- Telehealth or remote care certification preferred
- Proficiency with electronic health records and telehealth systems
- Strong critical thinking and decision-making abilities
- Excellent communication and interpersonal skills
- Ability to work independently while collaborating effectively
- Current BLS and ACLS certifications
