Responsibilities
- Provide telehealth assessments, patient education, and care coordination via virtual platforms
- Monitor patient vitals and health data remotely using advanced monitoring systems
- Collaborate with interdisciplinary teams to develop and implement care plans
- Document clinical encounters accurately in electronic health records (EHR)
- Respond to patient inquiries and triage urgent situations per protocols
- Participate in quality improvement initiatives and telehealth program development
Qualifications
- Active RN license in Florida (FL RN)
- BSN degree (MSN preferred)
- 3+ years clinical nursing experience
- Telehealth or remote care certification (e.g., CHTP)
- Proficiency with EHR systems and telehealth platforms
- Strong critical thinking and virtual communication skills
- BLS and ACLS certifications required
