Responsibilities
- Deliver high-quality virtual patient assessments via telehealth platform
- Develop and implement personalized care plans for chronic disease management
- Collaborate with interdisciplinary teams to coordinate patient care
- Document patient encounters accurately in EHR systems
- Provide health education and medication management guidance
- Respond to urgent patient inquiries within SLA timeframe
- Participate in quality improvement initiatives
Qualifications
- Active RN license in good standing (US state)
- Minimum 2 years clinical nursing experience
- Bachelor of Science in Nursing (BSN) preferred
- Telemetry or ICU experience highly desirable
- Proficiency with EHR systems (Epic, Cerner)
- Strong critical thinking and decision-making skills
- Excellent written and verbal communication abilities
- Ability to work independently in a remote environment
