Responsibilities
- Provide virtual patient assessments and clinical consultations via telehealth platform
- Coordinate care plans with multidisciplinary teams across specialties
- Monitor and interpret patient data from remote monitoring devices
- Deliver patient education and health coaching through virtual channels
- Maintain accurate electronic health records (EHR) documentation
- Respond to patient inquiries and triage emergent situations remotely
- Participate in quality improvement initiatives for remote care delivery
Qualifications
- Active RN license in Maryland (or compact state)
- BSN required; MSN preferred
- 3+ years acute care or telehealth nursing experience
- Proficiency with EHR systems and telehealth technology
- Strong critical thinking and remote communication skills
- BLS and ACLS certifications required
- Experience with chronic disease management preferred
