Responsibilities
- Deliver high-quality patient assessments and care coordination via telehealth platform
- Monitor patient progress and adjust treatment plans collaboratively with care teams
- Provide education and emotional support to patients and families remotely
- Maintain accurate electronic health records (EHR) documentation

Qualifications
- Active Texas RN license in good standing (compact license accepted)
