Responsibilities
- Deliver high-quality virtual patient assessments and care coordination via telehealth platform
- Manage patient records and ensure HIPAA-compliant documentation
- Collaborate with interdisciplinary teams to develop personalized care plans
- Provide patient education and health coaching for chronic conditions
- Respond to urgent virtual consultations within SLA timeframes

Qualifications
- Active California RN license (Compact License preferred)
- Minimum 2 years acute care or telehealth experience
- BLS/ACLS certification required (PALS preferred)
- Proficiency in telehealth platforms (e.g., Teladoc, Amwell)
- Strong clinical judgment and critical thinking abilities
- Exceptional communication skills for virtual patient interactions
