Responsibilities
- Conduct comprehensive virtual patient assessments via telehealth platform
- Develop and implement personalized care plans for chronic disease management
- Coordinate care transitions between providers and facilities
- Provide patient education on medications, treatments, and lifestyle modifications
- Document clinical encounters accurately in EMR system
- Triage urgent patient concerns and escalate appropriately
- Collaborate with multidisciplinary teams to optimize patient outcomes
Qualifications
- Active RN license in Indiana (or compact state)
- 2+ years clinical experience in med-surg, telehealth, or case management
- BLS certification required (ACLS preferred)
- Proficiency with telehealth platforms and EMR systems
- Exceptional communication and critical thinking skills
- Ability to work independently with minimal supervision
- Strong problem-solving abilities for remote patient scenarios
- Commitment to weekend availability (Sat/Sun shifts)
