Responsibilities
- Perform comprehensive telephone and video-based patient assessments to determine appropriate level of care.
- Evaluate patient symptoms, medical history, and current health status to provide accurate triage recommendations.
- Document all patient interactions and triage decisions in the Electronic Health Record (EHR) system accurately and in a timely manner.
- Facilitate appropriate referrals to specialists, urgent care, or emergency services when necessary.
- Collaborate with a multidisciplinary care team to ensure continuity of patient care.
- Provide health education and coaching to patients regarding disease prevention and wellness.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in California (or willingness to obtain one if remote).
- Minimum of 2-3 years of clinical nursing experience, preferably in triage, critical care, or urgent care.
- Valid Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Strong proficiency in electronic health record (EHR) systems, such as Epic or Cerner.
- Excellent verbal and written communication skills with a focus on empathy and clarity.
- Ability to remain calm and make sound clinical decisions under pressure.
