Responsibilities
- Personal Care Assistance: Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting, always maintaining a respectful and comfortable environment.
- Medication Reminders: Promptly remind clients to take their prescribed medications at scheduled times and report any missed doses or side effects to the Case Manager.
- Meal Preparation: Prepare nutritious, appetizing meals that meet specific dietary restrictions and preferences, ensuring proper nutrition for optimal health.
- Companionship: Provide emotional support and engage clients in meaningful conversations, hobbies, and recreational activities to reduce isolation and improve mental well-being.
- Light Housekeeping: Maintain a safe and clean living environment by performing light cleaning, laundry, and changing linens as needed.
- Transportation: Accompany clients to medical appointments, social events, or grocery shopping, ensuring safe travel.
- Health Monitoring: Observe and document any changes in a client's physical or mental health status and communicate concerns to the supervising nursing staff immediately.
Qualifications
- Education: High School Diploma or GED is required.
- Certifications: Valid CPR and First Aid certification is mandatory. A Minnesota Home Care Aide Certificate is preferred but not always required for entry-level.
- Experience: Previous experience as a CNA, PCA, Home Health Aide, or in a similar caregiving role is highly desirable.
- Driver's License: A valid, unrestricted Driver’s License and access to a reliable vehicle are required for this position.
- Communication Skills: Excellent verbal and written communication skills with the ability to listen actively and empathize with clients.
- Background Check: Must pass a comprehensive background check and drug screening.
