Responsibilities
- Monitor Patient Health: Observe patients' conditions, recording vital signs and reporting any changes to the nursing staff or physicians immediately.
- Assist with Daily Living: Help patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support with dignity and respect.
- Administer Care: Perform basic medical procedures including wound care, catheter care, and measuring intake and output under supervision.
- Documentation: Maintain accurate, up-to-date, and confidential patient records in the Electronic Health Record (EHR) system.
- Communication: Serve as a primary communicator between patients, families, and the interdisciplinary care team, ensuring all parties are informed of care plans.
- Emergency Response: Respond promptly to patient emergencies and assist in maintaining a safe, sanitary environment.
Qualifications
- Education: Current and valid State Certified Nursing Assistant (CNA) or Licensed Vocational Nurse (LVN) license required.
- Certifications: Basic Life Support (BLS) and Basic Arrhythmia (CPR) certification must be active.
- Experience: Minimum of 1-2 years of experience in a hospital or long-term care setting preferred.
- Skills: Strong interpersonal and communication skills; ability to read and interpret medical documents.
- Physical Requirements: Ability to lift, stand, and walk for extended periods; ability to assist patients with mobility aids.
