Responsibilities
- Provide personalized care to clients with complex medical needs in private residences
- Assist with ADLs including bathing, dressing, and medication management
- Monitor vital signs and document health status using electronic health records
- Coordinate with healthcare professionals to implement care plans
- Transport clients to medical appointments using company vehicles
- Perform light housekeeping and meal preparation
- Maintain strict confidentiality and HIPAA compliance
Qualifications
- Minimum 2 years professional home health experience
- Certified Nursing Assistant (CNA) or HHA certification required
- CPR/First Aid certification (or willingness to obtain)
- Valid Texas driver's license and clean driving record
- Ability to lift 50+ pounds and stand for extended periods
- Proficiency in electronic health record systems
- Spanish bilingual preferred (not required)
