Responsibilities
- Conduct comprehensive and accurate patient assessments via telephone, secure video, and secure messaging platforms.
- Analyze patient symptoms and medical history to determine the appropriate level of care and urgency.
- Make appropriate clinical triage decisions and facilitate referrals to urgent care centers, specialists, or emergency departments when necessary.
- Collaborate effectively with physicians, nurse practitioners, and care coordinators to ensure seamless patient continuity.
- Document all patient interactions and triage decisions accurately and timely in the Electronic Health Record (EHR) system.
- Maintain strict patient confidentiality and adhere to HIPAA regulations and company policies at all times.
- Provide patient education and support regarding self-care and preventive health measures.
Qualifications
- Education: Current and active Registered Nurse (RN) license in the State of New York.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Experience: Minimum of 3-5 years of clinical nursing experience, with specific experience in triage, emergency medicine, or telehealth preferred.
- Skills: Strong clinical judgment, critical thinking skills, and the ability to remain calm under pressure.
- Communication: Excellent verbal and written communication skills with a focus on empathy and clarity.
- Technology: Proficiency with electronic health record systems and telehealth technology platforms.
