Responsibilities
- Conduct comprehensive virtual patient assessments and health evaluations via telehealth platform
- Develop and implement personalized care plans for patients with chronic conditions
- Provide remote patient education and medication management support
- Collaborate with interdisciplinary teams to coordinate care services
- Document clinical encounters accurately in electronic health records (EHR)
- Respond to patient inquiries and triage emergent situations appropriately

Qualifications
- Valid New York State Registered Nurse license (RN)
- BSN or higher degree in Nursing (MSN preferred)
- Minimum 3 years of clinical nursing experience
- Telehealth or virtual care experience strongly preferred
- Certification in telehealth nursing or tele-ICU (desired)
- Strong proficiency with EHR systems and telehealth platforms
- Exceptional communication and critical thinking skills
