The Role: As a Triage Nurse, you will serve as the critical first point of contact for patient inquiries. You will assess patient symptoms via phone and virtual channels, ensuring the right level of care is provided efficiently and safely.
Responsibilities
- Perform comprehensive phone and telehealth triage assessments to evaluate patient symptoms and medical needs.
- Make critical clinical decisions regarding patient care pathways, including home care, urgent care, or emergency services.
- Document all patient interactions and assessments accurately and in compliance with HIPAA regulations.
- Communicate effectively with patients, families, and multidisciplinary care teams to ensure continuity of care.
- Manage high call volumes while maintaining a calm, professional demeanor under pressure.
- Collaborate with physicians and care coordinators to facilitate appropriate referrals.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the State of Pennsylvania.
- Minimum of 2 years of recent clinical nursing experience, preferably in a triage or telehealth setting.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Strong critical thinking skills and the ability to prioritize patient safety effectively.
- Excellent verbal and written communication skills.
- Proficiency with electronic health records (EHR) and secure messaging platforms.
