Responsibilities
- Deliver high-quality patient care in telehealth and remote monitoring settings
- Collaborate with interdisciplinary teams to develop and implement care plans
- Conduct virtual patient assessments and medication management
- Maintain accurate electronic health records (EHR) documentation
- Provide patient education and health coaching via telecommunication platforms
- Adhere to NJ Board of Nursing regulations and hospital protocols
- Participate in virtual quality improvement initiatives
Qualifications
- Active NJ RN license (or compact state license)
- Minimum 2 years of acute care experience
- BLS/ACLS certification required
- Experience with telehealth platforms (e.g., Teladoc, Amwell)
- Strong critical thinking and decision-making skills
- Excellent communication abilities for virtual interactions
- Flexibility to work rotating schedules including weekends
- Valid driver's license for occasional on-site visits
