Job Overview
As a Triage Nurse, you will utilize your clinical expertise to assess patient symptoms via phone and virtual platforms. You will play a critical role in decision-making, directing patients to the appropriate level of care, whether that is self-care at home, an urgent care visit, or emergency services.
Responsibilities
- Conduct comprehensive phone assessments to determine the severity of patient symptoms and recommend appropriate medical care pathways.
- Facilitate efficient patient flow by prioritizing appointments and emergency interventions based on clinical protocols.
- Provide expert education and advice to patients regarding home care, medication management, and preventative health strategies.
- Document all triage interactions accurately and in real-time within the Electronic Health Record (EHR) system.
- Collaborate closely with physicians and care managers to develop and refine individualized treatment plans.
- Maintain up-to-date knowledge of clinical guidelines, medical policies, and regulatory compliance standards.
Qualifications
- Active, unencumbered RN license issued by the California Board of Nursing.
- Minimum of 2-3 years of experience in emergency room, urgent care, or telehealth triage settings.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Strong communication skills with the ability to de-escalate tense situations and convey complex medical information clearly.
- Proficiency in using EMR/EHR systems (e.g., Epic, Cerner).
- Ability to work independently while strictly adhering to quality assurance and compliance standards.
