Responsibilities
- Assess patient conditions via telephone triage and determine appropriate level of care urgency
- Collaborate with physicians and multidisciplinary teams to coordinate patient care pathways
- Maintain accurate electronic medical records while adhering to HIPAA compliance standards
- Provide critical patient education on home care instructions and follow-up protocols
- Monitor and respond to changes in patient status using standardized assessment tools
- Participate in continuous quality improvement initiatives for triage processes
Qualifications
- Current California RN license with minimum 2 years emergency/urgent care experience
- BLS, ACLS, and TNCC certifications required
- Proficiency in EHR systems (Epic preferred)
- Exceptional critical thinking and decision-making abilities
- Strong communication skills with demonstrated cultural competency
- BSN or higher degree preferred
- Experience with telephone triage protocols
