Responsibilities
- Conduct rapid patient assessments via telehealth and in-person triage to determine medical urgency and appropriate care pathways
- Implement evidence-based triage protocols while maintaining HIPAA compliance and documentation standards
- Collaborate with multidisciplinary teams including physicians, specialists, and emergency services to ensure seamless care transitions
- Provide clear, empathetic communication to patients and families during high-stress medical situations
- Oversee patient flow management in emergency and urgent care settings to optimize resource utilization
- Maintain precise electronic health records (EHR) documentation using Epic systems
- Participate in continuous quality improvement initiatives for triage processes
Qualifications
- Active California RN license with minimum 3 years clinical experience
- BLS, ACLS, and TNCC certifications required (PALS preferred)
- Proven expertise in emergency department or urgent care triage
- Mastery of EHR systems (Epic experience highly valued)
- Exceptional critical thinking and decision-making abilities
- Strong emotional intelligence for de-escalating high-stress scenarios
- Spanish bilingual proficiency preferred
- BSN or higher degree from accredited institution
