Responsibilities
- Assess patient symptoms and medical histories to determine urgency and appropriate care level
- Provide telehealth and in-person triage services using evidence-based protocols
- Coordinate with physicians, specialists, and ancillary departments for seamless patient flow
- Document assessments and care plans accurately in Epic EHR system
- Educate patients on home care instructions and when to seek immediate attention
- Maintain compliance with HIPAA and Joint Commission standards
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Wisconsin (WI RN)
- Minimum 3 years emergency department or urgent care experience
- BLS, ACLS, and TNCC certifications required
- Proficiency in triage protocols (ESI, CTAS, or similar)
- Strong communication skills with diverse populations
- Ability to work efficiently in high-pressure situations
- Epic EHR experience preferred
- Bachelor of Science in Nursing (BSN) preferred
