Responsibilities
- Assess patient calls via phone and triage based on medical urgency
- Collaborate with physicians to determine appropriate care pathways
- Maintain accurate electronic health records (EHR) documentation
- Provide clear patient education and discharge instructions
- Coordinate with emergency services for critical cases
- Monitor patient conditions and escalate concerns promptly
- Support clinic workflow optimization initiatives
Qualifications
- Active RN license in Texas (or compact state)
- Minimum 2 years triage/ER experience
- BLS and ACLS certifications required
- Proficient in EHR systems (Epic preferred)
- Exceptional critical thinking and communication skills
- Ability to work efficiently in high-pressure environments
- Spanish fluency a significant plus
