Responsibilities
- Perform comprehensive patient assessments via phone and in-person, utilizing evidence-based protocols to determine appropriate level of care.
- Classify patient acuity levels accurately to prioritize urgent cases and ensure timely medical intervention.
- Document all patient interactions, assessments, and recommendations accurately and timely within the Electronic Medical Record (EMR) system.
- Collaborate closely with physicians, nurse practitioners, and care teams to facilitate seamless transitions of care and referrals.
- Provide patient education and self-care instructions to promote recovery and prevent readmission.
- Manage high call volumes efficiently while maintaining a professional, empathetic, and calming demeanor.
Qualifications
- Current and active Registered Nurse (RN) license in the state of Arizona.
- Minimum of 2 years of experience in a clinical or triage setting is preferred.
- Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) is required.
- Strong communication and interpersonal skills with the ability to de-escalate difficult situations.
- Proficiency with EMR systems (Epic, Cerner, or similar) is highly desirable.
- Ability to work flexible hours, including weekends and holidays, as required by the department.
