Responsibilities
- Perform Clinical Assessments: Evaluate patient symptoms via phone or secure messaging to determine the urgency of medical conditions.
- Direct Care Pathways: Make critical decisions regarding patient disposition (ER, Urgent Care, PCP, or Home Care) based on clinical guidelines.
- Provide Evidence-Based Advice: Offer accurate triage instructions and symptom management guidance to patients.
- Document Interactions: Accurately record all patient encounters, assessments, and advice given in the Electronic Health Record (EHR).
- Manage Patient Queues: Prioritize incoming requests based on clinical acuity to ensure timely response times.
- Collaborate with Providers: Communicate effectively with physicians and care managers to ensure continuity of care.
Qualifications
- Licensure: Valid, unencumbered RN license in the state of Texas.
- Certifications: BLS and ACLS certification required; TNCC preferred.
- Experience: Minimum of 2 years of nursing experience, with a focus on Triage, Urgent Care, or Emergency Department settings.
- Communication: Exceptional verbal and written communication skills; ability to de-escalate anxious patients effectively.
- Technology: Proficient in using Electronic Health Records (EHR) and telehealth platforms.
- Decision Making: Strong critical thinking and sound judgment in high-pressure situations.
