Responsibilities
- Conduct Patient Assessments: Perform comprehensive phone and virtual evaluations to determine the severity of patient symptoms.
- Triage & Triage Decisions: Utilize clinical judgment to prioritize patient needs and recommend appropriate next steps, including emergency care.
- Patient Education: Provide clear instructions on medication, home care, and lifestyle changes to ensure patient understanding.
- Documentation: Accurately record patient interactions, medical histories, and care plans in the Electronic Health Record (EHR).
- Referral Management: Coordinate referrals to specialists or primary care physicians as necessary.
- Quality Assurance: Participate in case reviews and quality improvement initiatives to maintain high standards of care.
Qualifications
- Licensure: Active and unencumbered Registered Nurse (RN) license to practice in the State of Hawaii.
- Experience: Minimum of 2-3 years of clinical nursing experience, preferably in a triage or call center setting.
- Communication Skills: Excellent verbal and written communication skills with a patient-centric approach.
- Technology Proficiency: Comfortable using electronic medical records and telehealth platforms.
- Certifications: Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications preferred.
- Flexibility: Ability to work varying shifts, including evenings, weekends, and holidays as needed.
