Responsibilities
- Perform comprehensive telephone and virtual patient assessments using evidence-based triage protocols
- Collaborate with physicians to prioritize patient cases and determine appropriate care pathways
- Document detailed clinical encounters in EMR systems with precision and timeliness
- Provide compassionate patient education on home care instructions and symptom management
- Coordinate care transitions between emergency departments, urgent care, and primary care
- Maintain strict HIPAA compliance and confidentiality standards
- Participate in continuous quality improvement initiatives
Qualifications
- Active Tennessee RN license with minimum 2 years clinical experience
- BLS certification; ACLS preferred
- Valid triage certification (ENPC, TNCC, or equivalent)
- Exceptional critical thinking and decision-making abilities
- Strong telehealth proficiency and digital literacy
- Proven ability to work independently while collaborating in team settings
- Excellent communication skills across diverse populations
- Flexibility to work evenings, weekends, and holidays as needed
