Responsibilities
- Conduct Patient Assessments: Evaluate patient symptoms via phone, email, or secure messaging to determine the urgency of care needs.
- Clinical Decision Making: Apply critical thinking to triage patients, distinguishing between self-care instructions, urgent care visits, and emergency room requirements.
- Provider Collaboration: Communicate findings clearly and concisely to physicians and healthcare providers to facilitate timely treatment plans.
- Patient Education: Provide detailed health education, medication instructions, and wellness advice to promote patient self-management and recovery.
- Documentation: Maintain accurate, up-to-date, and detailed medical records within the Electronic Health Record (EHR) system.
- Referral Coordination: Facilitate referrals to specialists or diagnostic services as needed to ensure continuity of care.
Qualifications
- License & Certification: Valid and active Registered Nurse (RN) license issued by the Texas Board of Nursing. BLS and ACLS certifications required.
- Experience: Minimum of 2-3 years of clinical nursing experience, preferably in a triage, urgent care, or telehealth setting.
- Communication Skills: Exceptional verbal and written communication skills with the ability to convey complex medical information in a clear, empathetic manner.
- Technical Proficiency: Comfortable navigating electronic health records (EHR) systems and telehealth platforms.
- Emotional Intelligence: Ability to remain calm under pressure and handle difficult patient interactions with professionalism and empathy.
- Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred.
