Responsibilities
- Perform comprehensive patient assessments via telephone and secure patient portal to determine urgency and appropriate level of care.
- Apply clinical protocols and nursing judgment to triage symptoms, provide home care instructions, and facilitate appointments with providers.
- Document all patient interactions and clinical findings accurately and timely in the EMR system (Epic preferred).
- Collaborate with physicians, nurse practitioners, and clinical staff to ensure continuity of care and resolve patient concerns.
- Educate patients on preventive health measures and manage chronic disease symptoms effectively.
- Escalate critical or complex cases to the on-call medical provider immediately.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Indiana.
- Minimum of 2 years of nursing experience, with at least 1 year in a triage or call center setting preferred.
- Associate’s Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
- Strong critical thinking skills and the ability to make rapid, accurate clinical decisions under pressure.
- Excellent verbal and written communication skills with a focus on patient empathy.
- Certifications: BLS (Basic Life Support) required; ACLS and TNCC preferred.
