As a Triage Nurse, you will be the critical link between patients and medical care, ensuring timely and accurate assessment of acute needs.
Responsibilities
- Perform comprehensive phone assessments to determine patient acuity and prioritize care needs.
- Consult with physicians and advanced practice providers to facilitate appropriate treatment plans.
- Document all triage encounters accurately and in compliance with regulatory standards.
- Provide patient education and resources to manage symptoms and prevent readmissions.
- Collaborate with the nursing staff and care coordination teams to ensure seamless transitions of care.
- Manage high-volume call queues efficiently while maintaining a calm and professional demeanor.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in California.
- Minimum of 2-3 years of nursing experience, with specific Triage or Urgent Care experience preferred.
- Certification in CEN (Certified Emergency Nurse) or similar preferred.
- Advanced knowledge of pathophysiology, pharmacology, and triage protocols.
- Strong verbal and written communication skills with a focus on patient-centered care.
- Ability to work in a fast-paced, high-pressure environment.
