Responsibilities
- Assess patient symptoms via phone triage to determine urgency and appropriate care level
- Collaborate with physicians and care teams to coordinate patient care pathways
- Maintain accurate electronic health records (EHR) documentation
- Educate patients on healthcare resources and home care instructions
- Manage patient flow and prioritize cases based on medical protocols
- Participate in continuous quality improvement initiatives
Qualifications
- Active North Carolina RN license (required)
