Responsibilities
- Assess Patient Acuity: Rapidly evaluate incoming patient calls and in-person visits to determine the severity of medical needs and prioritize care.
- Clinical Decision Making: Utilize clinical guidelines and protocols to recommend appropriate next steps, including home care, pharmacy interventions, or emergency department referrals.
- Documentation: Maintain accurate, timely, and thorough electronic health records (EHR) documenting triage assessments, interventions, and patient outcomes.
- Collaboration: Communicate effectively with physicians, nurse practitioners, and other healthcare professionals to ensure continuity of care.
- Patient Education: Provide clear, empathetic health education to patients and families regarding symptoms, medications, and self-care management.
- Quality Assurance: Participate in quality improvement initiatives and adhere to all state and federal healthcare regulations.
Qualifications
- Licensure: Current, unencumbered Registered Nurse (RN) license in the state of California.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) required; TNCC or CEN preferred.
- Experience: Minimum of 2-3 years of recent nursing experience, preferably in urgent care, emergency room, or telehealth triage settings.
- Skills: Strong critical thinking, excellent verbal communication skills, and proficiency in EMR systems (Epic, Cerner, etc.) are highly desired.
- Character: Professional demeanor with the ability to remain calm under pressure and manage high call volumes.
