Responsibilities
- Assess patient symptoms via phone, email, or secure messaging to determine urgency.
- Make clinical decisions to direct patients to appropriate levels of care (e.g., home care, ER, specialist).
- Document all patient interactions and triage decisions accurately in the EMR system.
- Collaborate with physicians, nurses, and care coordinators to ensure seamless patient transitions.
- Provide patient education and self-care instructions for minor ailments.
- Monitor and follow up on patient outcomes to ensure satisfaction and safety.
- Adhere to HIPAA regulations and hospital protocols strictly.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the state of California.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Minimum of 2 years of nursing experience, with specific experience in triage or urgent care preferred.
- Strong clinical judgment, critical thinking, and problem-solving skills.
- Excellent verbal and written communication abilities.
- Ability to remain calm and professional under pressure.
- Proficiency with electronic health records (EHR) systems.
