Responsibilities
- <ul><li>Provide high-quality personal care and assistance to patients with daily living activities, including bathing, dressing, and grooming.</li><li>Monitor and record vital signs, such as blood pressure, pulse, temperature, and respiration rates, and report any abnormalities to the RN.</li><li>Assist patients with mobility and transfers using proper body mechanics and assistive devices to ensure safety.</li><li>Serve meals and assist patients with feeding as needed, ensuring dietary restrictions are followed.</li><li>Maintain a clean, safe, and sanitary environment for patients in accordance with health department regulations.</li><li>Communicate effectively with patients, families, and interdisciplinary healthcare team members to ensure continuity of care.</li><li>Document patient care activities and changes in condition accurately in the Electronic Health Record (EHR).</li></ul>
Qualifications
- <ul><li>Current and valid California CNA or LPN license (must be in good standing with the state board).</li><li>Basic Life Support (BLS) and CPR certification from the American Heart Association (AHA) required.</li><li>High school diploma or GED equivalent required.</li><li>Strong interpersonal skills and the ability to demonstrate empathy and patience in a high-volume setting.</li><li>Ability to stand, walk, and lift up to 50 lbs for extended periods.</li><li>Must be able to pass a background check and drug screen prior to employment.</li><li>Previous experience in a hospital or long-term care facility is preferred but not required for immediate hires.</li></ul>
