Responsibilities
- Provide virtual patient assessments and care coordination via telehealth platforms
- Monitor and document patient vitals and health status remotely
- Assist patients with medication management and health education
- Collaborate with RNs and healthcare teams during virtual consultations
- Respond to patient inquiries and triage urgent situations
- Maintain accurate electronic health records (EHR) documentation
- Support discharge planning and care transitions
Qualifications
- Current Oregon CNA license in good standing
- Minimum 1 year clinical experience
- Proficiency with telehealth software (e.g., Teladoc, Amwell)
- Strong communication and problem-solving skills
- Ability to work independently with minimal supervision
- BLS/CPR certification required
- Experience with EHR systems (Epic or Cerner preferred)
