Responsibilities
- Perform remote patient assessments and monitor vital signs via secure video conferencing.
- Communicate effectively with patients, families, and the clinical care team to ensure continuity of care.
- Document patient interactions, symptoms, and treatment plans accurately in our EHR system.
- Provide patient education and lifestyle coaching to promote wellness and prevent readmission.
- Collaborate with physicians and nursing staff to develop and implement care plans.
- Ensure strict adherence to HIPAA regulations and remote work security protocols.
Qualifications
- Certified Nursing Assistant (CNA) or Licensed Vocational Nurse (LVN) certification required.
- Active, unencumbered nursing license in the state of Texas (or willing to obtain).
- Proven experience in patient care and basic life support (BLS) certification.
- Strong computer literacy and proficiency with telehealth software.
- Excellent communication skills and a compassionate bedside manner (even remotely).
- Reliable high-speed internet connection and a private workspace.
