Responsibilities
- Provide high-quality virtual patient assessments, diagnoses, and treatment plans via telehealth platform
- Monitor and manage chronic conditions through remote patient monitoring systems
- Coordinate care with multidisciplinary teams and community resources
- Document patient encounters accurately in EHR systems while maintaining HIPAA compliance
- Deliver health education and lifestyle coaching to patients through virtual channels
- Respond to urgent patient inquiries and triage emergent situations appropriately
- Participate in continuous quality improvement initiatives for remote care delivery
Qualifications
- Current, unrestricted RN license in Oregon (compact license accepted)
- Minimum 2 years of clinical nursing experience in telehealth or ambulatory care
- BLS certification required; ACLS preferred
- Proficiency with telehealth platforms and EHR systems (e.g., Epic, Cerner)
- Exceptional communication skills with ability to build rapport virtually
- Strong clinical judgment and ability to work independently
- Experience with remote patient monitoring technology
- Master's degree in Nursing or relevant specialty certification preferred
