Responsibilities
- Provide virtual patient assessments via telehealth platforms
- Assist with medication management reminders
- Coordinate care plans between patients and clinical teams
- Document patient interactions in electronic health records (EHR)
- Conduct wellness check-ins via video/phone
- Collaborate with RNs on patient care coordination
- Respond to patient inquiries within SLA timeframes
Qualifications
- Active Georgia CNA certification
- High school diploma or equivalent
- Basic computer proficiency (EHR systems)
- Strong verbal communication skills
- Empathy and patient-centered mindset
- Ability to pass background/drug screening
- Reliable high-speed internet connection
