Responsibilities
- Provide telehealth consultations and patient assessments via secure video platforms
- Monitor and interpret patient data from remote monitoring devices
- Collaborate with multidisciplinary teams to develop patient care plans
- Document patient encounters accurately in electronic health records (EHR)
- Educate patients on chronic disease management and medication adherence
- Triage urgent cases and coordinate emergency services when necessary

Qualifications
- Active RN license in good standing (any US state)
- Minimum 2 years of clinical nursing experience
- Experience with telehealth platforms or remote patient monitoring

- Proficiency in EHR systems and healthcare IT tools
- BLS certification required; ACLS preferred
