Responsibilities
- Provide telehealth patient monitoring and support during night hours
- Assist with virtual medication reminders and health tracking
- Document patient interactions and vital signs in EHR systems
- Coordinate with on-site nursing teams for urgent interventions
- Conduct wellness checks via video calls for assigned patients
- Respond to patient inquiries and provide emotional support
Qualifications
- Active CNA certification in Texas
- Minimum 1 year clinical experience
- Strong communication and tech proficiency
- Ability to work independently during night shifts
- Valid CPR/BLS certification
- Pass background check and drug screening
