Responsibilities
- Conduct comprehensive patient health assessments via secure video conferencing and telephone channels.
- Document patient medical history, symptoms, and treatment plans accurately in electronic health records (EHR).
- Collaborate closely with physicians, nurse practitioners, and care coordinators to develop and adjust personalized care plans.
- Provide detailed health education, medication management guidance, and lifestyle coaching to patients.
- Monitor patient progress through follow-up calls and virtual visits, escalating critical issues immediately.
- Maintain strict adherence to HIPAA regulations and state-specific nursing practice acts while working remotely.

Qualifications
- Active and unencumbered Registered Nurse (RN) license issued by the Texas Board of Nursing.
- Graduation from an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
