Responsibilities
- Provide direct patient care and assistance with Activities of Daily Living (ADLs) such as bathing, dressing, and grooming.
- Monitor vital signs (blood pressure, temperature, pulse) and report any abnormalities to the nursing staff immediately.
- Assist with patient transfers, mobility, and positioning to ensure safety and prevent falls.
- Document patient care accurately and thoroughly in electronic health records (EHR).
- Maintain a clean, organized, and therapeutic environment in accordance with infection control standards.
- Communicate effectively with patients, families, and the interdisciplinary healthcare team.
- Administer medications as directed and supervised by registered nurses.
Qualifications
- Certified Nursing Assistant (CNA) or Licensed Practical Nurse (LPN) license valid in the State of Illinois.
- Current Basic Life Support (BLS) certification from the American Heart Association.
- Previous experience in a long-term care facility, hospital, or home health setting is preferred.
- Strong interpersonal and communication skills with a patient-centered approach.
- Ability to stand for extended periods and lift up to 50 lbs as required.
- Compassionate demeanor with a commitment to patient dignity and respect.
- Must pass a background check and drug screen prior to employment.
