Responsibilities
- Provide virtual patient assessments and care coordination
- Monitor vital signs and document health metrics digitally
- Assist with telehealth appointments and medication management
- Collaborate with RNs and physicians via secure platforms

Qualifications
- Active California CNA certification required
- 1+ years clinical experience in geriatric/acute care
- Proficient with telehealth platforms (Zoom, Epic, etc.)
- Strong digital communication skills
- CPR/BLS certification current
- Ability to pass background/drug screening
- Reliable high-speed internet and quiet workspace
- Spanish bilingual preferred
