Responsibilities
- Provide virtual patient support via telehealth platforms for daily living assistance
- Document patient care accurately in electronic health records (EHR) systems
- Coordinate with RNs and care teams to implement care plans remotely
- Conduct wellness checks and medication reminders via video calls
- Assist patients with technology setup for virtual appointments
- Collaborate with social workers to address patient needs
- Maintain HIPAA compliance in all digital communications
Qualifications
- Active Pennsylvania CNA certification in good standing
- Minimum 1 year of clinical experience (remote preferred)
- Proficiency with telehealth platforms and EHR systems
- Strong written/verbal communication skills
- Ability to work independently with minimal supervision
- Reliable high-speed internet and quiet home office setup
- Basic computer troubleshooting skills
- Valid Pennsylvania driver's license (for occasional in-person visits)
